University-based COVID-19 Vaccination Program【Starting on April 18】

    This is to inform you that Soka University will be conducting the 3rd dose of the University-based COVID-19 Vaccination Program for free (under the framework of the Workplace COVID-19 Vaccination Program). The schedule and other related information are as follows:

    Authorized Absence:
    Considering the fact that the vaccination program is conducted during the class operation period and there may be some side effects after vaccination, Soka University and Soka Women’s College students can apply for authorized absences on the vaccination day, the following day and 2 days after the vaccination. (Please make sure to attach any document proving your vaccination when filing an application).

    Schedule:
    April 18 (Mon) through April 22 (Fri), April 25 (Mon), and April 26 (Tue)

    Applicable to:
    Soka University students (students of graduate and undergraduate programs, Soka Women’s College students, and
    Distance Learning Division students); Soka University faculty and staff (including adjunct faculty members, etc.); and the family members of Soka University full-time faculty and staff.
    ※Please note that this 3rd dose of vaccine is available only for those who completed their 2nd dose at least 6 months ago.
    ※“The family members of Soka University full-time faculty or staff ” need to be 18 years of age or older in order to be eligible.

    Time and Availability: Time: 1:00 – 6:00 pm
    Available Doses: 410 doses per day (410 doses per day * 7 days = 2,850 doses in total)
    Manufacturer: Takeda/Moderna vaccine
    Venue: 1F lobby, Ikeda Auditorium, Soka University
    What to Bring: Vaccination Voucher
    Student ID or Faculty ID (or any other ID card such as a driver’s license to identify the applicant)
    Health Insurance Card

    About the Vaccination Voucher:
    For vaccination, you must bring a Vaccination Voucher for COVID-19 issued by the local government.
    Please make sure that your address on the Vaccination Voucher for COVID-19 is the same as it appears on your certificate of residence (jyuminhyo).     About the Health Insurance Card:
    Please make sure to bring your health insurance card with you, because it may be needed when you feel sick after vaccination.
