This is to inform you that Soka University will be conducting the University-based COVID-19 Vaccination Program (Workplace COVID-19 Vaccination Program).

[Notes]

• If you wish to be vaccinated, please carefully read through the “Instructions for the COVID-19 Vaccination” and the following guidelines, and then apply for the vaccination.
• Soka University students are requested to consult with their parents regarding the vaccination in advance. If you are under the age of 20, please make sure to obtain your guardian’s consent to be vaccinated.
• Please note that the vaccination is only for those who wish to be vaccinated and is not compulsory.

≪Vaccinations are Available for≫
※Soka University students (graduate, undergraduate, Soka Women’s College students, and including Distance Learning Division students）; Soka University faculty and staff （including part-time lecturers, etc.）; family members of Soka University full-time faculty and staff; individuals who work on the Soka University campus; individuals affiliated with educational institutions near Soka University (i.e. the faculty, staff, and students of Kogakuin University and Tokyo Junshin University, and the faculty and staff of Toyo Elementary School, Nakano Kita Elementary School, Shimizu Elementary School, Daikyu Elementary School, and Kounohara Junior High School.)
※If you have not received a Vaccination Voucher from your local government, you will not be able to be vaccinated.
※“The family members of Soka University full-time faculty or staff members” need to be 12 years old or older and live with Soka University full-time faculty or staff.

≪Type of Vaccine≫

• Vaccine manufactured by Moderna/Takeda

≪Vaccination Period≫

• 1st Dose: August 24（Tue）～September 3（Fri）*excluding August 29（Sun）
• 2nd Dose: September 21（Tue）～October 1 (Fri) *excluding September 26（Sun）

※Times are 2:00 pm- 6:00 pm
※We are planning to vaccinate about 440 people per day, and in total about 4,400 people during the vaccination period.

≪Venue≫

• Matsukaze Center 3F, Soka University

≪Regarding Reservations≫

• Please make a reservation using the reservation website. The URL for the reservation website will be announced through the portal site for individuals affiliated with Soka University, and the others will be contacted by their respective institutions for how to make a reservation.
• Using the vaccination website, please make a reservation only for the first dose.
• It is necessary to be vaccinated twice. The second dose is given after an interval of 4 weeks from the first dose. Specifically, the second dose is to be given on the same day of the week and time as the first, and given 4 weeks after the first dose. Therefore, when making an appointment, please carefully consider your schedule for the dates of both the first and second dose of the vaccination.
• Reserving multiple appointment dates is prohibited.
• Reservations for the second dose will be explained when you visit the venue for the first dose.

≪Start of reservations≫

• Reservations will begin 10:00 am on August 12 (Thurs)

≪Reservation deadline, cancellations, and changes≫

• Reservation deadline is 1 day (24 hours) before the vaccination day.
• Once you have made a reservation, you must attend and be vaccinated. The vaccine is precious and in limited supply, so it must not be wasted.
• If you need to cancel or change your appointment with a compelling reason, please do so at least 2 days (48 hours) prior to your appointment date through the reservation website.

≪Items to bring on the vaccination day≫ *If you forget to bring any of these items listed below to the venue, you will not be able to receive the vaccination that day.
①Student/faculty/staff ID card (for those who are not students/faculty/staff, please bring an employee ID card or another form of identification from your employer, or other acceptable means of identification)
②Reservation Confirmation (You will receive it by e-mail after your reservation is confirmed. Please show it on your smartphone screen or bring a printout of it with you at the reception desk on the day of your appointment.)
③Completed Pre-vaccination Screening Questionnaire for COVID-19 Vaccine Form (of Japanese Format)
Please refer to the website of the Ministry of Health, Labor and Welfare (MHLW) below for instructions on how to fill out the Pre-vaccination Screening Questionnaire for COVID-19 Vaccine Form in various languages.

As noted in the website, you must fill in the Japanese form sent by the local government, by referring to the translations available in the website. Please fill in all the information in the bold frame in advance and bring it with you to the venue.
Please make sure that your address on the form is the same as it appears on your certificate of residence (jyuminhyo). Please note that there are some people whose address on the certificate of residence is different from their current address. Please double-check before completing the form.
④Vaccination Voucher for COVID-19 issued by the local government.

≪Other≫

• If the appointment slots are completely booked, please research and avail yourself of the "Joint Vaccination by the Tokyo Metropolitan Government and Universities" or the "Mass Vaccination by Local Governments" options.
• If traveling by car, please use the parking area for visitors at Ryokufu Parking and Midori No Oka Parking
