3) Register your temperature daily!

On the "Immunization Questionnaire on Measles and Rubella," "Medical Interview Sheet," and "Physical Examination Certificate": 2. Immunization Questionnaire on Measles and Rubella
　It is mandatory to submit this document prior to admission, therefore, please do so before entering in Japan.
　In 2007, measles were rampant among High School and College students in Japan. In order to prevent group contagion, students are required to submit this questionnaire compulsorily.

3. Medical Interview Sheet
　From now, you will live in a completely different environment for a long period. Even healthy people can feel sick considering these abrupt changes. In order to provide the best support possible, please cooperate by filling in this questionnaire.

※Should be completed by a doctor.

　Please visit the hospital and ask a doctor to fill out this ‘Physical Examination Certificate’ after undergoing the necessary examinations.

　If you wish to live in the dormitory, please read the following Dormitory Terms of Agreement carefully in advance.
　You will be asked to fill out the pledge form at the orientation after your arrival in Japan.

　You will be asked to fill out this form during the orientation after your arrival in Japan.

Permission to engage in other activities
　If your residence status is "Student," you will need to apply for "Permission to Engage in Activity Other Than That Permitted under the Status of Residence Previously Granted" in order to engage in various activities in Japan such as part-time work and volunteer work. The application can be made during the entry inspection at the immigration booth at the airport, and the permit will be issued on the spot. Please make sure to apply at the airport.

　Once the permit is granted, it will be printed on the lower back of your residence card. Before departure, please print and fill out the following application form and submit it during the entry inspection at the airport.
